Our goal at Riverside Eye and Laser Specialists is to make your visit as pleasant and hassle-free as possible. Prior to your appointment, we encourage you to fill out the new patient questionnaire via our online patient portal. This information will be given to you when you schedule your appointment. If you are unable to complete the online questionnaire prior to your appointment, then you will need to arrive to your appointment 30 minutes early so that you can have ample time to fill out the appropriate forms. Please make sure you also have your driver’s license and insurance cards with you at your appointment so we can make a copy of them. We will also need a list of all the medications you are taking for your chart.
